Insurance Information and Payments

Doctor Hatfield is a Preferred Provider for Blue Cross/Blue Shield, Health Advantage and many other health insurance plans.  In addition, we also participate in Medicare and Medicaid programs.

At the time of your office visit, please present your insurance or Medicare ID card to the receptionist.  You will be asked to complete a medical history questionnaire and sign an authorization allowing us to file your insurance.

As a courtesy to our patients, we will file a claim with Medicare and most private insurance companies.  Your insurance policy may cover all or a portion, or none of your medical expenses.  Cost of medical services and treatments rendered are ultimately the responsibility of the patient and NOT the insurance company.  Any expenses incurred represent a contract between the patient and our clinic.

It is our policy that all co-payments and any unmet deductibles must be paid at the time of the office visit.  We accept Mastercard, Visa, American Express and Discover.

Office visits and procedure charges are based on the usual and customary fees for Arkansas.  Charges vary, based on the level of difficulty, number of conditions to evaluate, and the degree of cognitive and surgical skills involved.  Fees also vary depending on whether it is an office consultation, new patient or established patient visit.

Many procedures are deemed "cosmetic" and are not covered by insurance policies.  The patient will be responsible for the full amount of the fee at the time of the procedure.

There will be a $25.00 administration fee for any returned checks.
